Japanese vehicles used to be of extremely high quality, but that started going down in the late 1980s. Now Toyota relies on its reputation more than anything. The fact that we Americans lease a new vehicle every two years isn't helping matters. Did your Four Runner have a V6? Here's what Toyota truck V6 engines liked to do from 1989 to 1995 (possibly longer):

The design is a cast iron block with aluminum heads. The heads expand and contract faster than the block. The head studs and gaskets are designed to accomodate this. The problem is part of one or both of the gaskets tends to get dragged into the combustion chamber when the heads cool down. On startup, this piece gets burned and begins to disintegrate. Eventually the rupture gets to one of the water passages and BAM! root beer float. Time between failures is influenced in part by heating/cooling cycles.
